Animal surrender forms
At Blue Iris Farm, we understand that sometimes, difficult circumstances may arise where you are no longer able to care for your beloved animal companion. That's why we offer an animal surrender program to help find loving homes for animals in need. Our surrender process is designed to be as smooth and stress-free as possible for both you and your animal friend.
To begin the surrender process, simply fill out our Surrender Application Form. This form provides us with important details about your animal, such as their health history, behavior, and any special needs they may have. Our team of compassionate experts will review your application and contact you to discuss the next steps.
Once your application is approved, we'll ask you to sign our Surrender Contract, which outlines the terms & conditions of the surrender agreement. This contract ensures that your animal will receive the best possible care and find a loving forever home.
